Johnny died. The Yakuza killed him.
Molly tells Case about it in Neuromancer, when she's climbing around in Straylight.
She came back to their place and this little vat-grown ninja was leaving. He looked like Hideo, and Molly knew what she would find when she came back to their room.
Johnny was just sitting there, his mouth slightly open, like he'd just thought of something to say.

I awlays kind of wanted to see Johnny and Molly: The Lost Episodes, a sort of Bonny and Clyde, Pumpkin and Honeybunny for the cyber-set.

I don't think Bill should necessarily backstep to revisit that old material, but a pastiche might be interesting.
